What is the Cardio-Renal-Metabolic (CRM) Health Review?

The Cardio-Renal-Metabolic (CRM) is a programme to review long-term health conditions and preventing future illness.

It focuses on three key areas:

  • Heart and blood vessel health (cardio)
  • Kidney health (renal)
  • Metabolic health, including diabetes, weight, cholesterol and blood pressure

Many serious conditions — such as heart disease, stroke, kidney disease and diabetes complications — can develop slowly over time without obvious symptoms.

The purpose of this programme is to identify risks early and take action before problems develop.

Why am I being contacted?

You are being invited because your medical record shows that you have one or more long-term health conditions, or risk factors, that may benefit from regular review.

Based on your medical history, patients are grouped into:

  • High risk
  • Moderate risk
  • Lower risk

This helps us provide the right level of care and monitoring for each patient.

Regular reviews allow us to:

  • Check that your condition is well controlled
  • Detect early signs of complications
  • Review medications if needed
  • Offer advice and support to improve your health
  • Reduce the risk of hospital admission or serious illness

What will happen next?

The process is simple and usually involves three steps:

Step 1 — Complete a short health questionnaire

You will receive a secure link to a website questionnaire.

This helps us collect important information before your appointment, including:

  • Lifestyle information
  • Mental wellbeing
  • Some health parameters eg Height, Weight, Waist size
  • Family history
  • Health Confidence questions
  • Any concerns you may have

Completing this in advance allows your appointment to be more focused and efficient.

Step 2 — Attend for tests and measurements

You will be invited for some routine checks (This will be by a SELF BOOKING link via SMS)

  • Blood pressure
  • Blood tests
  • Urine test
  • Weight and waist measurement
  • Foot check (for patients with diabetes)

Not all patients will need all tests.

Step 3 — Review results and plan next steps (this may be telephone based or Face to Face)

Once your results are available, we will:

  • Review your overall risk
  • Discuss any changes needed
  • Adjust medication if required
  • Arrange follow-up if necessary

Many patients will simply continue with routine monitoring.

Yes.

Is this urgent?

No.

This is a routine health review designed to prevent future problems.

However, it is important to take part when invited.
